  1. Complete the MSM PA Program Preceptor Application (hardcopy attached or electronic versions available at: https://morehouseschoolofmedicine.formstack.com/forms/pa_program_preceptor_application
    • Upload required documents (CV, Medical License, Board Certification)
  1. Complete the MSM PA Program Letter of Intent

    • Provide your annual availability for student placements.

    3. Read, sign, and return the Morehouse School of Medicine Affiliation Agreement (if applicable)
